To file a claim against the City of Santa Monica for an injury or for property damage, a claim form must be completed. The form can be obtained from the City Clerk’s Office on the first floor of City Hall, 1685 Main Street, or by calling (310) 458-8211. It is also available on-line (in PDF) at www.smgov.net. Hours are Monday through Thursday from 7:30 a.m. to 5:30 p.m., and 8 a.m. to 5 p.m. on alternate Fridays.
